Student Affiliation Agreements

What is an Affiliation Agreement?

A written agreement between the campus and a Host that establishes the expectation of the Host and the student while they are interning or training at the Host site. This affiliation agreement is required for a student to be covered by SUNY's general and professional liability insurance.

  • Parties to the agreement are SUNY, on behalf of the Campus, and the Host.
  • SUNY provides liability insurance if the applied learning experience is one of the academic disciplines covered by SUNY’s insurance (see list of disciplines).

Why are Affiliation Agreements Important?

    • Protects both SUNY and students.
    • Clearly communicates the Campus’ expectations for both the Host and student(s) (with course curriculum) if student is receiving credit.
    • Provides liability insurance.

Contracts

*Note: Affiliation Agreements cannot be more than five (5) years old).

Affiliation Agreements are to be filled out using the AAMC Uniform Clinical Training Affiliation Agreement with corresponding AAMC Implementation Letter or the Non-Clinical Affiliation Agreement (if unsure which to use, contact Jacob Irving). Any agreements that deviate from the templates above (outside of any “fill-in-the-blank” data (i.e. host name and address information, etc.)) will need to be reviewed by SUNY General Counsel. 

It is campus policy that the Host site provides a signature prior to SUNY Cortland signing. Electronic and digital signatures are not acceptable. See Signature Types.

Certificate of Insurance

  • Liability
    • SUNY procures a commercial liability insurance policy to cover the defense and indemnification of students who participate in applied learning experiences in certain academic areas. This liability policy is intended to cover students who are named as a party in a legal action arising from their participation in a program, namely to pay any judgment or settlement on behalf of students as a result of “bodily injury” or “property damages” attributable to the students and covered by the policy, including payment of associated legal expenses.
    • To obtain insurance, SUNY requires the following:
      1. The student must receive academic credit or be meeting a course- or program-requirement by participating in the clinical training program;
      2. The experience must be supervised by the Host and coordinated by a faculty member (this does not mean that a faculty member must be on-site);
      3. There must be an Affiliation Agreement in place between SUNY and the Host; and
      4. The program must be approved by the insurance company for coverage under the policy.
  • Student Teachers
    • Education Law requires local school districts to provide student teachers with liability insurance coverage.

If multiple disciplines, then you need (1) COI request form for each discipline.
