Determine Which Verification Form You Need

2019-2020 Verification Forms
(Summer 2019, Fall 2019 and Spring 2020)

Answer the following questions to determine which verification form to submit:

  1. Were you born before January 1, 1996?
  2. Are you a veteran of the U.S. Armed Forces?
  3. Will you be working on a master's program during the school year 2019-2020?
  4. As of today are you married?
  5. Are you an orphan or ward of the court, or were you a ward of the court until age 18?
  6. Do you have any dependents (other than your children or spouse) who live with you and who received more than half of their support from you, now through June 30, 2020?
