Student Information

For appointments or information:

  • The quickest, easiest way to make appointments is to use the Student Health Portal, which you can use to make, view or cancel appointments anytime.
  • Phone: 607-753-4811, select option 6 to speak with the triage nurse to make an appointment (or listen to the options for other information); Call us if you don't find an appropriate appointment time on the Student Health Portal.

Be sure to cancel any appointment that you are unable to keep.  SHS has instituted a $25 charge for any missed appointments.  You can cancel anytime, 24/7, on the student health portal.  

Why did we do this?  Approximately 10% of our scheduled appointments do not keep them.  Not canceling prevents us from being able to offer those appointment times to other students who need them and then many of those students need to seek care elsewhere.  By canceling your appointment if you are not going to use it, we can offer those times to someone else and save many students a trip to urgent care or the emergency department.


Clinical service hours during the academic year are:

  • 8:15 a.m. - Noon –Visits by Appointment
  • Noon - 1 p.m. – Nurse service only
  • 1-4:30 p.m. – Visits by Appointment

New Students

New Students are required to complete the following forms:

  • Health History Form: A brief overview of your health history
  • Immunization information:  Required for ALL students, dates/proof of immunity against Measles, Mumps, Rubella, and Meningococcal disease
  • Tuberculosis Screening Form: A screening questionnaire for risks for tuberculosis infection
  • Under 18 Consent Form- Only required for students that will be under 18 when they arrive on campus.

All intercollegiate athletes are required to:

  • have a physical examination 
  • have a sickle cell trait test performed
  • carry health insurance that covers injuries up to $90,000

PRIOR to the beginning of practice or tryouts.

International Students

All international students must complete the following health requirements for attending SUNY Cortland:

  • A physical examination before coming to campus
  • A Tuberculin skin test (PPD)- Only if you are from, or spent time in, a high risk country as defined by the CDC
