Completing a Care Report
Please complete a Care Report if you are concerned for a student's well-being and suspect that there may be a need to connect them to assistance. All of the information included in the Care Report will reviewed thoroughly. Care and Outreach Services will conduct outreach and follow-up, to assist with connecting students to the appropriate resources and support services.
EMERGENCY PROTOCOL: If a student is displaying threatening behaviors or is a danger to themselves or others CALL 911 or contact the University Police Department at 607-753-2111.

Who should make a referral?
Faculty, staff, parents, and students are all encouraged to submit concerns to Care and Outreach Services. If you are worried, take action and be an advocate for a student who you feel needs assistance.
How do I make a referral?
- Provide a student with contact information for Care and Student Outreach
- Self-advocate
- Complete the Care Report
