Do you or someone in your family work on a farm, or work in any of the industries listed below?

• Agricultural (such as hay, dairy, fruit or vegetable crops, poultry, nursery/greenhouse, etc.)
• Fishing 
• Logging, harvesting, or initial processing of trees.
• Work at a food processing plant, (such as meat or poultry processing plants, packing fruits or vegetables, etc.)

Have you moved at any point in the last three years?
