Exempt Application and Information

Category I: Exempt Review

The term "exempt" does not mean that IRB review is not required, and it does not indicate that the investigator is exempt from following the federal regulations/guidelines for human subjects research. All human subjects research must follow the federal rules, regulations, and guidelines; and, all human subjects research must be reviewed by the IRB. OHRP decision charts may help you decide if you should apply for exempt review. The IRB holds the final responsibility for classifying the level of review. If you would like assistance with identifying the appropriate level of review for your research, contact the IRB (email).

If you are certain that your research meets the requirements for exempt review, you can access the application instructions and application here.

When you have completed the application form, attach and email to: irb@cortland.edu.

Original copies of signature-bearing documents (e.g., permission letters, Appendix A) are to be sent through U.S. Mail, intercampus mail, or by fax to the IRB. If Appendix B is applicable to your research, include Appendix B as an attachment with your emailed application.
