Determine Which Verification Document You Need
To determine which verification form to download and print, please answer the following questions:
- Were you born before January 1, 1992?
- Are you a veteran of the U.S. Armed Forces?
- Will you be working on a master's program during the school year 2015-2016?
- As of today are you married?
- Are you an orphan or ward of the court, or were you a ward of the court until age 18?
- Do you have any dependents (other than you children or spouse) who live with you and who received more than half of their support from you, now through June 30, 2016?
