Medical Paperwork Requirements for Camp:
All campers must submit proof of health insurance; proof of physical exam (within one year of camp start date) and immunization record through our new online camper portal, Camp Docs. A link to register with CampDocs will be emailed to you within one or two weeks of registration. Any questions specific to uploading or submitting medical paperwork can be directed to the CampDocs support team at support@campdoc.com or 734-619-8300. If you have not yet received an email from CampDocs, please check the spam/junk mail folders of the email account you listed on your registration.
**Please check the email account provided at registration for all CampDoc correspondence**
**All medical paperwork is due to be uploaded to CampDocs three days prior to the start of camp.**
**We will NOT be accepting any of the required medical forms in paper form via fax, mail, or in person.**
