Photo ID Procedure

  1. Fill out the membership application in the Club Rec brochure and bring it to the Recreational Sports Office located at 1120 Park Center between the following hours: 8:00 AM to 5:00 PM, Monday through Friday.

  2. Pay fees and a photo I.D. card will be made for you in the Recreational Sports Office.

  3. Members who already have photo I.D. cards from last year may use the same I.D. this year. Just bring the I.D. card along with this membership application to the Recreational Sports Office, pay your fees, and a new validation sticker will be placed on your card.

  4. All participants must return to the Recreational Sports Office at the beginning of each semester to have their I.D. revalidated, unless they pay for the calendar year. Memberships for the calendar year must be purchased in the fall. Otherwise, memberships must be purchased by the semester or for the summer.

  5. Replacement fee for lost I.D. card is $5.00.
